Cases of invasive pneumococcal disease in Hong Kong have risen following the pandemic. Serotype 3 remains the most common, while the proportion of cases caused by serotype 15A has also increased.
The Pneumococcal 21-valent vaccine is indicated for adults aged 18 years and above to help prevent pneumococcal pneumonia and invasive pneumococcal disease caused by the serotypes covered by the vaccine.
According to the Hong Kong Department of Health, serotypes 3, 15A, and 23A are the predominant serotypes causing invasive pneumococcal disease (IPD) in the local elderly population. Pneumococcal 21‑valent vaccine is currently the vaccine that covers these three serotypes.

Vaccine Features
Suitable for Adults Aged 18 Years and Above
Designed to address adult vaccination needs, the vaccine is indicated for adults aged 18 years and above. Older adults, individuals with chronic medical conditions, and others who may be at increased risk of pneumococcal disease should first undergo an assessment by a healthcare professional, taking into account their health status, vaccination history and vaccination needs.
Covers 21 Pneumococcal Serotypes
Pneumococcal 21‑valent vaccine covers 21 pneumococcal serotypes, including serotypes 3, 15A and 23A, providing targeted protection to help prevent pneumococcal pneumonia and invasive pneumococcal disease caused by the relevant serotypes.
According to Hong Kong surveillance data for 2023–2025, serotype 3 accounted for approximately 39.9% of invasive pneumococcal disease cases, making it the most common serotype. Serotypes 15A and 23A were also among the more frequently reported serotypes, accounting for approximately 8.0% and 4.0% of cases, respectively.
These percentages describe the serotype distribution among invasive pneumococcal disease cases across all age groups in Hong Kong. They are not adult-specific figures or measures of vaccine effectiveness.
Conjugate Vaccine Technology
The vaccine combines pneumococcal polysaccharide antigens with a carrier protein to stimulate an immune response against the relevant serotypes and help develop immunological memory, providing an immunological basis for the prevention of disease caused by the serotypes covered by the vaccine.
Prevention of Pneumococcal Pneumonia and Invasive Disease
Helps prevent pneumococcal pneumonia and invasive pneumococcal disease, including bacteraemia, sepsis and meningitis, caused by the serotypes covered by the vaccine.
Single-Dose Administration
Administered to adults as a single 0.5 mL dose by intramuscular injection.
Pre-vaccination Assessment
Individuals who have previously received other pneumococcal vaccines should provide their vaccination card or records so that a doctor can assess subsequent vaccination arrangements.
Before vaccination, inform your healthcare professional if any of the following applies:
- Seniors aged 65 or above, smokers, and individuals with diabetes or chronic heart or lung disease are at high risk of pneumococcal disease and should proactively inform healthcare professionals for assessment of vaccination needs and timing.
- History of allergic reactions: Individuals who have experienced allergic reactions to any vaccine component, including diphtheria toxoid, should inform their healthcare professional. Those allergic to these components should not receive the vaccine.
- Acute illness or infection: Vaccination should generally be postponed in individuals with an acute severe febrile illness or acute infection.
- Bleeding risk: Individuals taking anticoagulants, or who have a low platelet count or a blood-clotting disorder, require assessment of the risk of bleeding or bruising following intramuscular injection.
- Impaired immune function: Individuals who are immunocompromised or receiving immunosuppressive treatment require individual assessment, as their immune response to vaccination may be reduced.
- Pregnancy or breastfeeding: A doctor should assess the potential benefits and risks before vaccination.
Post-vaccination Precautions
Common reactions include injection-site pain, fatigue, headache and muscle pain. Most local and systemic reactions are mild to moderate and short-lived, typically lasting no more than three days.
Seek immediate medical attention if you develop symptoms suggestive of a severe allergic reaction, such as difficulty breathing, swelling of the face or lips, or hives.

- Customers are required to make an appointment for and complete the first dose of the vaccine within 90 days from the date of purchase. If the vaccine requires only a single dose, it must be completed within this timeframe. All vaccinations (multi-dose) must be completed within 6 months from the date of the first dose; otherwise, they will become void.

- All customers must present identification documents and the successful payment confirmation email during registration at the centre to be eligible for the service.
