400;\">\u4ee5\u4e0b\u689d\u6b3e\u53ca\u7d30\u5247\u5217\u51fa\u4e86\u7ba1\u7406\u6b64\u91ab\u7642\u5361\u8a08\u5283\u7684\u898f\u5247\uff08\u300c\u6b64\u689d\u6b3e\u53ca\u7d30\u5247\u300d\uff09\u3002\u6b64\u689d\u6b3e\u53ca\u7d30\u5247\u53ea\u9069\u7528\u65bc\u9999\u6e2f\u7279\u5225\u884c\u653f\u5340\uff08\u300c\u9999\u6e2f\u300d\uff09\u5883\u5167\uff0c\u4e26\u53d7\u9999\u6e2f\u6cd5\u5f8b\u7ba1\u8f44\u3002\u5168\u4ec1\u6709\u6b0a\u96a8\u6642\u4fee\u6539\u6b64\u689d\u6b3e\u53ca\u7d30\u5247\u6216\u66ab\u505c\u6216\u7d42\u6b62\u6b64\u91ab\u7642\u5361\u8a08\u5283\u800c\u6bcb\u9808\u53e6\u884c\u901a\u77e5\u3002\u5982\u6709\u4efb\u4f55\u722d\u8b70\uff0c\u5168\u4ec1\u4fdd\u7559\u6700\u7d42\u6c7a\u5b9a\u6b0a\u3002\u5982\u6b64\u689d\u6b3e\u53ca\u7d30\u5247\u4e4b\u82f1\u6587\u53ca\u4e2d\u6587\u7248\u672c\u5728\u5167\u5bb9\u4e0a\u6709\u4efb\u4f55\u6b67\u7570\uff0c\u5747\u4ee5\u82f1\u6587\u7248\u672c\u70ba\u6e96\u3002<\/span><\/p><ol \u53c3\u52a0\u91ab\u7642\u5361\u8a08\u5283<\/span><\/p><p><span style=\"font-weight: 400;\">\u4f60\u53ef\u4ee5\u901a\u904e\u5168\u4ec1\u53c3\u52a0\u6b64\u91ab\u7642\u5361\u8a08\u5283\uff0c\u4e26\u76f4\u63a5\u900f\u904e\u6b64\u91ab\u7642\u5361\u8a08\u5283\u7684\u7db2\u7ad9\u767b\u8a18\uff0c\u6216\u6191\u5168\u4ec1\u6240\u63d0\u4f9b\u7684\u514c\u63db\u78bc\uff0c\u900f\u904e\u6b64\u91ab\u7642\u5361\u8a08\u5283\u7684\u7db2\u7ad9\u514c\u63db\u6703\u7c4d\u3002\u4e00\u65e6\u6210\u529f\u767b\u8a18\uff0c\u5168\u4ec1\u5c07\u5411\u4f60\u767c\u9001\u78ba\u8a8d\u96fb\u90f5\uff0c\u4ea6\u4ee3\u8868\u4f60\u540c\u610f\u53ca\u63a5\u53d7\u6b64\u689d\u6b3e\u53ca\u7d30\u5247\u3002<\/span><\/p><p><span style=\"font-weight: 400;\">\u5168\u4ec1\u4fdd\u7559\u6700\u7d42\u6c7a\u5b9a\u662f\u5426\u63a5\u53d7\u5176\u767b\u8a18\u7684\u6b0a\u5229\u3002<\/span><\/p><p><span style=\"font-weight: 400;\">2.3 \u91ab\u7642\u5361\u6703\u7c4d\u8a08\u5283<\/span><\/p><p><span style=\"font-weight: 400;\">\u4f60\u53ef\u4ee5\u900f\u904e\u6b64\u91ab\u7642\u5361\u8a08\u5283\u7684\u7db2\u7ad9\u67e5\u770b\u4f60\u6240\u767b\u8a18\u7684\u6703\u7c4d\u8a08\u5283\u3002\u6703\u54e1\u53ef\u65bc\u6703\u7c4d\u6709\u6548\u671f\u5167\u7121\u9650\u6b21\u4ee5\u512a\u60e0\u50f9\u9322\u4f7f\u7528\u76f8\u61c9\u7b49\u7d1a\u4e4b\u670d\u52d9\u3002<\/span><\/p><p><span style=\"font-weight: 400;\">\u4f60\u53ef\u4ee5\u81ea\u884c\u6c7a\u5b9a\u4f7f\u7528\u8207\u5426\u53ca\u4f7f\u7528\u54ea\u4e9b\u512a\u60e0\u53ca\u670d\u52d9\u3002\u6709\u95dc\u512a\u60e0\u53ca\u670d\u52d9\u7531\u5168\u4ec1\u6216\u4efb\u4f55\u5168\u4ec1\u6307\u5b9a\u7684\u5408\u4f5c\u5925\u4f34\u63d0\u4f9b\uff0c\u800c\u6bcf\u9805\u512a\u60e0\u53ca\u670d\u52d9\u90fd\u53ef\u80fd\u53d7\u5176\u4ed6\u689d\u6b3e\u53ca\u7d30\u5247\u7d04\u675f\u3002\u5982\u679c\u4f60\u9078\u64c7\u4f7f\u7528\u8a72\u9805\u512a\u60e0\u53ca\u670d\u52d9\uff0c\u5247\u53d7\u8a72\u9805\u512a\u60e0\u53ca\u670d\u52d9\u7684\u9644\u52a0\u689d\u6b3e\u53ca\u7d30\u5247\u7d04\u675f\u3002\u5982\u679c\u6b64\u689d\u6b3e\u53ca\u7d30\u5247\u8207\u9644\u52a0\u7684\u689d\u6b3e\u53ca\u7d30\u5247\u6709\u4efb\u4f55\u4e0d\u4e00\u81f4\u4e4b\u8655\uff0c\u5247\u4ee5\u5f8c\u8005\u70ba\u6e96\u3002\u5168\u4ec1\u53ef\u4ee5\u96a8\u6642\u6c7a\u5b9a\u64a4\u56de\u3001\u66ab\u505c\u6216\u4fee\u6539\u4efb\u4f55\u9644\u5e36\u512a\u60e0\u53ca\u670d\u52d9\u800c\u6bcb\u9808\u53e6\u884c\u901a\u77e5\u3002<\/span><\/p><p><span style=\"font-weight: 400;\">2.4 \u6703\u671f<\/span><\/p><p><span style=\"font-weight: 400;\">\u6703\u7c4d\u8a08\u5283\u70ba\u671f\u5341\u4e8c\u500b\u6708\u3002\u5982\u679c\u4f60\u4e0d\u7e8c\u8a02\u91ab\u7642\u5361\u6703\u54e1\u8cc7\u683c\uff0c\u5247\u6703\u5728\u6703\u671f\u5c46\u6eff\u65e5\u81ea\u52d5\u5931\u53bb\u6703\u54e1\u8cc7\u683c\u3002\u5982\u679c\u4f60\u7533\u8acb\u7e8c\u6703\uff0c\u5168\u4ec1\u6709\u6b0a\u6c7a\u5b9a\u662f\u5426\u63a5\u53d7\u3002<\/span><\/p><ol 400;\">\u5982\u679c\u4f60\u6c92\u6709\u63d0\u4f9b\u5168\u4ec1\u6216\u4efb\u4f55\u5168\u4ec1\u7684\u5408\u4f5c\u5925\u4f34\u6240\u9700\u7684\u8cc7\u6599\uff0c\u5247\u4f60\u5c07\u7121\u6cd5\u4f7f\u7528\u670d\u52d9\u3002<\/span><\/p><p><span style=\"font-weight: 400;\">\u4f60\u53ef\u80fd\u9700\u8981\u900f\u904e\u4e92\u806f\u7db2\u63d0\u4f9b\u4e00\u4e9b\u8cc7\u6599\u3002\u7db2\u7d61\u50b3\u8f38\u7684\u6578\u64da\u5b89\u5168\u662f\u4e0d\u80fd\u7d55\u5c0d\u4fdd\u8b49\u7684\u3002\u5168\u4ec1\u4e0d\u4fdd\u8b49\u4f60\u901a\u904e\u6b64\u91ab\u7642\u5361\u8a08\u5283\u7684\u7db2\u7ad9\u6216\u5408\u4f5c\u5925\u4f34\u7db2\u7ad9\u5728\u7dda\u50b3\u8f38\u7684\u4efb\u4f55\u8cc7\u6599\u7684\u5b89\u5168\u6027\u3002\u4f60\u9700\u8981\u81ea\u884c\u627f\u64d4\u901a\u904e\u4e92\u806f\u7db2\u63d0\u4f9b\u7684\u4efb\u4f55\u8cc7\u6599\u6240\u884d\u751f\u7684\u5b89\u5168\u98a8\u96aa\u3002<\/span><\/p><p><span style=\"font-weight: 400;\">3.2 \u9a57\u8b49\u4f60\u63d0\u4f9b\u7684\u8cc7\u6599<\/span><\/p><p><span style=\"font-weight: 400;\">\u5168\u4ec1\u6709\u6b0a\u9a57\u8b49\u4f60\u63d0\u4f9b\u7684\u8cc7\u6599\uff0c\u5305\u62ec\u8207\u4f60\u63d0\u4f9b\u7684\u8cc7\u6599\u4e2d\u63d0\u8ff0\u7684\u4eba\u54e1\uff08\u4f8b\u5982\u91ab\u751f\uff09\u806f\u7e6b\u3002\u4f60\u540c\u610f\u4ed6\u5011\u8207\u5168\u4ec1\u8a0e\u8ad6\u4f60\u4e26\u5411\u5168\u4ec1\u62ab\u9732\u4f60\u7684\u500b\u4eba\u8cc7\u6599\uff08\u5305\u62ec\u654f\u611f\u8cc7\u6599\u548c\u75c5\u6b77\uff09\u3002\u5168\u4ec1\u4e5f\u53ef\u80fd\u6703\u8981\u6c42\u4f60\u63d0\u4f9b\u66f4\u591a\u8cc7\u6599\u6216\u8b49\u64da\uff0c\u4ee5\u9a57\u8b49\u76f8\u95dc\u8cc7\u6599\u3002<\/span><\/p><p><span style=\"font-weight: 400;\">3.3 \u4f7f\u7528\u670d\u52d9\u7684\u8cac\u4efb<\/span><\/p><p><span style=\"font-weight: 400;\">\u4f60\u9700\u8981\u81ea\u884c\u627f\u64d4\u4f7f\u7528\u670d\u52d9\u7684\u98a8\u96aa\u3002\u901a\u904e\u6b64\u91ab\u7642\u5361\u8a08\u5283\u63d0\u4f9b\u7684\u670d\u52d9\u4ee5\u53ca\u8cc7\u8a0a\u548c\u6750\u6599\u53ef\u80fd\u4e26\u4e0d\u9069\u5408\u6240\u6709\u91ab\u7642\u5361\u7684\u6703\u54e1\u3002\u4f60\u5728\u4f7f\u7528\u6709\u95dc\u512a\u60e0\u53ca\u670d\u52d9\u6642\u5fc5\u9808\u76e1\u529b\u78ba\u4fdd\u81ea\u5df1\u7684\u5b89\u5168\u4e26\u907f\u514d\u5c0d\u81ea\u5df1\u9020\u6210\u50b7\u5bb3\u3002<\/span><\/p><p><span style=\"font-weight: 400;\">\u6b64\u91ab\u7642\u5361\u8a08\u5283\u4e0d\u69cb\u6210\u91ab\u7642\u5efa\u8b70\uff0c\u4e26\u4e14\u4e0d\u80fd\u66ff\u4ee3\u91ab\u7642\u5efa\u8b70\u6216\u6cbb\u7642\u3002\u5728\u52a0\u5165\u6b64\u91ab\u7642\u5361\u8a08\u5283\u6216\u4f7f\u7528\u4efb\u4f55\u670d\u52d9\u4e4b\u524d\uff0c\u4f60\u61c9\u5c31\u4f60\u7684\u500b\u4eba\u60c5\u6cc1\uff08\u5305\u62ec\u4efb\u4f55\u5065\u5eb7\u6216\u91ab\u7642\u72c0\u6cc1\uff09\u8aee\u8a62\u91ab\u751f\u6216\u7368\u7acb\u65bc\u6b64\u6703\u54e1\u8a08\u5283\u7684\u9069\u7576\u5c08\u696d\u4eba\u54e1\u3002<\/span><\/p><p><span style=\"font-weight: 400;\">\u901a\u904e\u6b64\u91ab\u7642\u5361\u8a08\u5283\u63d0\u4f9b\u7d66\u4f60\u7684\u8cc7\u8a0a\u548c\u6750\u6599\u53ef\u80fd\u4e0d\u662f\u57fa\u65bc\u6700\u65b0\u7684\u767c\u73fe\u6216\u7814\u7a76\uff0c\u4e5f\u4e0d\u662f\u70ba\u4efb\u4f55\u7279\u5b9a\u7684\u4eba\u5ea6\u8eab\u8a02\u9020\u7684\u3002\u53ef\u80fd\u6709\u5065\u5eb7\u7814\u7a76\uff0c\u4f7f\u901a\u904e\u5168\u4ec1\u6216\u5168\u4ec1\u7684\u5408\u4f5c\u5925\u4f34\u63d0\u4f9b\u7684\u8cc7\u6599\u904e\u6642\u6216\u56e0\u5176\u4ed6\u539f\u56e0\u4e0d\u9069\u7528\u3002\u5728\u672a\u4e8b\u5148\u8aee\u8a62\u8a72\u91ab\u751f\u6216\u5c08\u696d\u5065\u5eb7\u4eba\u54e1\u7684\u60c5\u6cc1\u4e0b\uff0c\u4e0d\u5f97\u56e0\u4f7f\u7528\u6b64\u91ab\u7642\u5361\u8a08\u5283\u7684\u670d\u52d9\u800c\u5ffd\u8996\u6216\u66f4\u6539\u91ab\u751f\u6216\u5176\u4ed6\u5065\u5eb7\u5c08\u696d\u4eba\u54e1\u5411\u4f60\u63a8\u85a6\u7684\u4efb\u4f55\u6cbb\u7642\u65b9\u6cd5\u3002<\/span><\/p><p><span style=\"font-weight: Members of all plans can enjoy access to relevant services and benefit within the service provider network at discounted prices for unlimited times within the membership period.<\/span><\/p><p><span style=\"font-weight: 400;\">You can decide whether or not and which, if any, of the benefits and services to use. Relevant benefits and services are provided by TRINITY or TRINITY\u2019s designated partners, and each benefit and service may be subject to other terms and conditions. If you choose to use this benefit and service, you are bound by the additional terms and conditions of the benefit and service. If there is any inconsistency between these Terms and Conditions and those additional terms and conditions, the latter shall prevail. If you use a benefit or service from TRINITY\u2019s designated partners or otherwise interact with them, you:<\/span><\/p><ul><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">agree to the partner\u2019s terms and conditions applicable to the benefit or service and must comply with those terms;<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">consent to disclose your personal and sensitive information by TRINITY to the partner and vice versa;<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">do so at your own risk;<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">agree that any benefit or service you obtain from TRINITY\u2019s partners are supplied directly by the partners and not TRINITY; and<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">must resolve any issues with the benefit or service directly with the partners and not TRINITY.<\/span><\/li><\/ul><p><span style=\"font-weight: 400;\">TRINITY is not responsible or liable for and TRINITY does not guarantee, endorse or recommend:<\/span><\/p><ul><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">any of TRINITY\u2019s partners\u2019 qualifications or expertise;<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">any of TRINITY\u2019s partners\u2019 actions, products or services including whether they are merchantable or fit for any particular purpose or provided with due care and skill;<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">any information or advice provided by any of TRINITY\u2019s partners or through the Program;<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">any of the contents of TRINITY\u2019s partners\u2019 websites or other platforms; and<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">the availability of any of the products or services from any of TRINITY\u2019s partners.<\/span><\/li><\/ul><ol start=\"5\"><li><b> Termination of the Medical Card<\/b><\/li><\/ol><p><span style=\"font-weight: 400;\">You may terminate your Medical Card by notifying TRINITY via 21927022 or send an email to bd.team@trinitymedical.com.hk. If your Medical Card membership is terminated, you will no longer be entitled to use any services under this Program. TRINITY will not refund any membership fee for the remaining term.<\/span><\/p><p><span style=\"font-weight: 400;\">In the event you choose to terminate your membership, note that such termination may not release you from the performance of any other agreement, obligation or liability on your part with or owed to any other third party. TRINITY shall bear no responsibility in communicating your termination to such third party.<\/span><\/p><p><span style=\"font-weight: 400;\">In the event you terminate your Medical Card membership and wish to re-apply for membership, TRINITY reserves the right to decide whether to accept your application.<\/span><\/p><p><span style=\"font-weight: 400;\">TRINITY has the sole discretion to, in whole or in part, with or without notice, at any time unilaterally terminate the Program, or terminate your membership.<\/span><\/p><p><span style=\"font-weight: 400;\">Membership will automatically be terminated in the event of a member\u2019s demise.<\/span><\/p><ol start=\"6\"><li><b> TM Aesthetics<\/b><\/li><\/ol><p><span style=\"font-weight: 400;\">Some benefits and services are provided by TM Aesthetics. Privacy Statement of this Program, available at the website of this Program, sets out how TRINITY collects, uses, handles and discloses your personal information and other important information.<\/span><\/p><p><span style=\"font-weight: 400;\">By providing information to TRINITY or other representatives, or by continuing your relationship with TRINITY and otherwise interacting with TRINITY, you confirm that you have been notified of the matters and consent to the collection, use, disclosure and handling of personal information as described in the Privacy Statement available on the website of this Program, as updated from time to time.<\/span><\/p><p><span style=\"font-weight: 400;\">You must ensure the accuracy of the personal information provided to TRINITY.
