\u91ab\u7642\u5361\u6703\u54e1\u8cc7\u683c<\/p><p>\u7533\u8acb\u6b64\u91ab\u7642\u5361\u8a08\u5283\u5fc5\u9808\u7b26\u5408\u4ee5\u4e0b\u6240\u6709\u689d\u4ef6\uff1a<\/p><ul><li>\u64c1\u6709\u9999\u6e2f\u5c45\u6c11\u8eab\u4efd\u8b49\u3001\u4e2d\u83ef\u4eba\u6c11\u5171\u548c\u570b\u5c45\u6c11\u8eab\u4efd\u8b49\u6216\u8b77\u7167\uff1b<\/li><li>\u5e74\u6eff \/>\u4f60\u9700\u8981\u81ea\u884c\u627f\u64d4\u4f7f\u7528\u670d\u52d9\u7684\u98a8\u96aa\u3002\u901a\u904e\u6b64\u91ab\u7642\u5361\u8a08\u5283\u63d0\u4f9b\u7684\u670d\u52d9\u4ee5\u53ca\u8cc7\u8a0a\u548c\u6750\u6599\u53ef\u80fd\u4e26\u4e0d\u9069\u5408\u6240\u6709\u91ab\u7642\u5361\u7684\u6703\u54e1\u3002\u4f60\u5728\u4f7f\u7528\u6709\u95dc\u512a\u60e0\u53ca\u670d\u52d9\u6642\u5fc5\u9808\u76e1\u529b\u78ba\u4fdd\u81ea\u5df1\u7684\u5b89\u5168\u4e26\u907f\u514d\u5c0d\u81ea\u5df1\u9020\u6210\u50b7\u5bb3\u3002\u6b64\u91ab\u7642\u5361\u8a08\u5283\u4e0d\u69cb\u6210\u91ab\u7642\u5efa\u8b70\uff0c\u4e26\u4e14\u4e0d\u80fd\u66ff\u4ee3\u91ab\u7642\u5efa\u8b70\u6216\u6cbb\u7642\u3002\u5728\u52a0\u5165\u6b64\u91ab\u7642\u5361\u8a08\u5283\u6216\u4f7f\u7528\u4efb\u4f55\u670d\u52d9\u4e4b\u524d\uff0c\u4f60\u61c9\u5c31\u4f60\u7684\u500b\u4eba\u60c5\u6cc1\uff08\u5305\u62ec\u4efb\u4f55\u5065\u5eb7\u6216\u91ab\u7642\u72c0\u6cc1\uff09\u8aee\u8a62\u91ab\u751f\u6216\u7368\u7acb\u65bc\u6b64\u6703\u54e1\u8a08\u5283\u7684\u9069\u7576\u5c08\u696d\u4eba\u54e1\u3002<\/p><p>\u901a\u904e\u6b64\u91ab\u7642\u5361\u8a08\u5283\u63d0\u4f9b\u7d66\u4f60\u7684\u8cc7\u8a0a\u548c\u6750\u6599\u53ef\u80fd\u4e0d\u662f\u57fa\u65bc\u6700\u65b0\u7684\u767c\u73fe\u6216\u7814\u7a76\uff0c\u4e5f\u4e0d\u662f\u70ba\u4efb\u4f55\u7279\u5b9a\u7684\u4eba\u5ea6\u8eab\u8a02\u9020\u7684\u3002\u53ef\u80fd\u6709\u5065\u5eb7\u7814\u7a76\uff0c\u4f7f\u901a\u904e\u5168\u4ec1\u6216\u5168\u4ec1\u7684\u5408\u4f5c\u5925\u4f34\u63d0\u4f9b\u7684\u8cc7\u6599\u904e\u6642\u6216\u56e0\u5176\u4ed6\u539f\u56e0\u4e0d\u9069\u7528\u3002\u5728\u672a\u4e8b\u5148\u8aee\u8a62\u8a72\u91ab\u751f\u6216\u5c08\u696d\u5065\u5eb7\u4eba\u54e1\u7684\u60c5\u6cc1\u4e0b\uff0c\u4e0d\u5f97\u56e0\u4f7f\u7528\u6b64\u91ab\u7642\u5361\u8a08\u5283\u7684\u670d\u52d9\u800c\u5ffd\u8996\u6216\u66f4\u6539\u91ab\u751f\u6216\u5176\u4ed6\u5065\u5eb7\u5c08\u696d\u4eba\u54e1\u5411\u4f60\u63a8\u85a6\u7684\u4efb\u4f55\u6cbb\u7642\u65b9\u6cd5\u3002<br Medical Card Program &#8211; Terms and Conditions<br \/><br \/><\/p><p>1. General terms and conditions<br \/><br \/><\/p><p>The \u201cIAPA&amp;Trinity\u201d Medical Card Program (the \u201cProgram\u201d) is provided and managed by Trinity Medical Centre Limited (\u201cTRINITY\u201d).<br \/><br \/><\/p><p>The following terms and conditions set out the rules governing this Program (the \u201cTerms and Conditions\u201d). These Terms and Conditions are applicable within Hong Kong Special Administrative Region (\u201cHong Kong\u201d) only and governed by the laws of Hong Kong. TRINITY reserves the right to amend these Terms and Conditions or suspend or discontinue the Program at any time without prior notice. All decisions made by TRINITY shall be final and binding. Relevant benefits and services are provided by TRINITY or TRINITY\u2019s designated partners, and each benefit and service may be subject to other terms and conditions. If you choose to use this benefit and service, you are bound by the additional terms and conditions of the benefit and service. If there is any inconsistency between these Terms and Conditions and those additional terms and conditions, the latter shall prevail. If you do not renew your membership, you will cease to be a member on the expiry date. You must answer all TRINITY\u2019s and TRINITY\u2019s designated partners\u2019 questions honestly and ensure that all information you provide is true, complete, accurate and not misleading. You must provide TRINITY and TRINITY\u2019s designated partners with all information including anymedical conditions that might be relevant to the benefits and services before you use them.<\/div><p>If you do not provide the information that TRINITY and TRINITY\u2019s designated partners require, you will not be able to use the benefits and services.<br \/>You may be required to provide certain information online. No data transmission over the Internet can be guaranteed as totally secure. TRINITY does not warrant and cannot ensure the security of any information which you transmit online whether via the website of the Program or the partners\u2019 websites. TRINITY will not refund any membership fee for the remaining term.<br \/>In the event you choose to terminate your membership, note that such termination may not release you from the performance of any other agreement, obligation or liability on your part with or owed to any other third party.
